Application Checklist & Important Dates

Application Checklist
As a part of the application process, the following items are required and need to be electronically attached with the application upon submission.  An application is considered not complete until received;

1. Completed Application Form through the SLCS website with payment of $200 non-refundable application fee
2. Current immunization records signed/stamped by a physician
3. Student’s original or certified copy of Birth Certificate
4. Copy of the student’s Baptismal Certificate
5. First Communion Certificate, if applicable
6. Previous and current year report cards are required for all grade levels, except PreK applicants, only when the child did not previously attend an educational institution
7. Previous and current year standardized test scores (Grades 1st -8^th)
8. Handwritten Essay by the student, one page in length, discussing the advantages and disadvantages of being a part of the St. Laurence School community (Grades 5th - 8^th)
9. Two teacher recommendations are required for all grade levels, except PreK applicants, only when the child did not previously attend an educational institution
10. Parish and Family Life Form
11. Screening conducted by St. Laurence Catholic School – Date will be scheduled upon receipt of a completed application.

1. Grade of “C” or higher in all academic areas,
2. Standardized test scores showing the student is at grade level or higher for reading, math, and language.
3. Behavior rating of satisfactory or better
